IN-NETWORK ESTIMATED COST
If you have a high deductible plan, the evaluation may cost up to $1850 and feedback may cost up to $170
OUT-OF-NETWORK ESTIMATED COST
The evaluation may cost up to $4600 and feedback will cost $275
INFORMATION ABOUT AUTHORIZATIONS
Some insurance plans require prior authorization (e.g., HMOs)
You are responsible for contacting your insurance company to find out if a prior authorization is required. If you do not obtain authorization before your appointment, you will be responsible for the total bill. See above for cost estimates.

Hours vary per provider. Typical hours of operation are Monday – Friday 9:00am – 4:30pm.
