Thank you for referring patients to our practice!  We want to make the referral process as convenient and efficient as possible to reduce the workload on administrative staff and patients.  We have found that the following procedures reduce wait times for patients and administrative burden on physicians and support staff.

When initially referring a patient, please fax the following information to Jessica Spina, at fax number 732-317-8994:

• Referral order (which can be a script or treatment request)

• Patient information: name, phone number and insurance information (which can be on the referral order or separate)

• Last visit note(s)

• Relevant labs, medications, tests and imaging reports including: labs/blood work, EEG, carotid doppler, neurological exam, imaging including head CT, MRI, MRA, PET, SPECT, etc.,

Once we have this information, we will reach out to your patient to facilitate scheduling.  We have found that receiving this information immediately upon referral accelerates scheduling and reduces the back and forth of record requests.
